Who is Eligible?

This program is designed for individuals who meet the following requirements. You must: 

  • Be eligible for MO HealthNet (Missouri Medicaid)

  • Be 18 years of age or older

  • Have a physical disability that requires assistance with daily living.

  • Be able to direct your own care (or have a designated representative who can).

  • Be assessed and approved to receive services.
