Choice Should Not Feel Complicated

Consumer Directed Services allows the person receiving care to choose a trusted caregiver, including a family member or friend, when program requirements are met.

That choice is meaningful, but the process can feel unfamiliar at first.

Families may wonder where to start, what paperwork is needed, how to transfer from another agency, or who handles the next step. Valentine's CDS helps carry that process so the client and family are not left trying to figure it out alone.

We help with referrals, transfers, required paperwork, and communication around getting services started.

The goal is simple: care directed by the client, supported by the family, and organized with steady follow-through.

The Client Chooses the Caregiver

In CDS, the client is the employer.

That means the person receiving care has a voice in who supports them at home. A caregiver may be a daughter, son, grandchild, trusted friend, or another person chosen by the client.

Valentine's CDS does not assign the caregiver.

The client chooses. We help support the process around that choice.

Care Remains Directed at Home

CDS is not traditional agency-assigned care.

The client remains at the center of the arrangement. Care is built around the person’s routine, preferences, and daily needs inside the home.

For Chosen Caregivers

A chosen caregiver carries real responsibility inside the home.

They may already know the person’s routine, preferences, and way of communicating. That familiarity matters.

Valentine's CDS supports the process with onboarding guidance, timekeeping expectations, and ongoing communication when questions come up.

Caregiver pay starts at $15 per hour, with rates reviewed based on the care arrangement and program requirements.

Who May Qualify

CDS may be available to individuals who meet program requirements and need assistance with daily living.

General requirements may include:

  • Eligibility for MO HealthNet
  • Age 18 or older
  • A physical disability requiring assistance with daily living
  • Ability to direct care, or support from a designated representative
  • Approval through the required assessment process
